CrosaCore LLC, doing business as CrosaCore, operates this website. This policy explains what information the website collects, how it is used, and the choices available to you.
This policy covers the public website. Information you provide once you become a client is handled under the separate agreements and notices provided during intake.
1. Who this policy covers
This policy applies to visitors to www.crosacore.com and to people who contact CrosaCore through the website form, by email, or by phone in response to the website.
CrosaCore is a private physical therapy practice located at 711 W 38th St, Suite F-4, Austin, Texas.
2. Information collected through this website
CrosaCore collects only the information you choose to provide. Through the contact form, that may include:
- Your name
- Your email address or phone number
- Your preferred method of response
- The category of your question
- The content of your message
- How you heard about CrosaCore, if you choose to answer
- Whether you consented to receive text messages
The form asks you not to include symptoms, diagnoses, medications, or other detailed health information. Please follow that guidance.
You may also contact CrosaCore directly by phone, text, or email without using the form.
3. Information collected automatically
This website does not use analytics software, advertising trackers, or marketing cookies. CrosaCore does not build visitor profiles and does not track your browsing across other websites.
Like nearly all websites, the hosting provider may keep standard server logs that can include IP addresses, browser type, and pages requested. Those logs are generated by the hosting provider for security and reliability rather than by CrosaCore for marketing.
Some pages link to third-party services, such as online scheduling. Those services may set their own cookies once you are on their platform. See section 6.
This site loads its typefaces from Google Fonts. That means your browser requests the font files from Google, and Google receives your IP address as part of that request. No cookie is set and nothing is stored by CrosaCore. If the fonts fail to load, the site falls back to typefaces already on your device.
4. How information is used
Information you submit is used to:
- Respond to your question or request
- Schedule or discuss a consultation, evaluation, or appointment
- Provide administrative support such as billing questions, forms, or receipts
- Send appointment or service text messages, if you consented to them
- Keep records CrosaCore is required or reasonably expected to keep
CrosaCore does not sell your information, rent it, or share it with advertisers.
5. When information is shared
Information may be shared only in limited situations:
- With service providers that operate necessary systems, such as email, scheduling, payment processing, or text messaging. These providers receive only what they need to perform their function.
- With your direction or authorization, for example when you ask CrosaCore to coordinate with another provider.
- When required by law, legal process, or a lawful request from a public authority.
- To protect the safety, rights, or property of a person or of CrosaCore.
6. Third-party services
This website links to independent services that operate under their own terms and privacy practices. These may include:
- PT Everywhere, used for scheduling the Pain Confidence Consultation and physical therapy evaluations
- Square or another processor, used for payments
- Email and text messaging providers
- Secure forms and records systems used during care
- Google, for the business profile and reviews, and for the typefaces this site uses
When you follow a link to one of these services, their privacy policy governs what happens there. CrosaCore does not control their cookies, retention practices, or security.
7. Health information and this website
The website contact form is intended for administrative and general questions. It is not a secure clinical communication system.
Please do not send detailed health information through the website form, ordinary email, or ordinary text message. That includes symptoms, diagnoses, medical history, medications, imaging, treatment records, or insurance documents.
When CrosaCore needs clinical information from you, it will be collected through a secure system identified to you at the time.
Information you share once you become a client is handled under the agreements and notices provided during intake rather than under this website policy.
8. How long information is kept
General inquiries from people who do not go on to receive care are kept only as long as needed to respond and to maintain a reasonable record of the conversation, and are then deleted periodically.
Records connected to professional services are retained for the period required by applicable law and professional standards.
Text messaging consent and opt-out records are kept for as long as needed to honor your preferences and to document that consent was given.
9. Security
CrosaCore takes reasonable steps to protect the information it holds, including limiting access and using established service providers.
No website, email system, or internet transmission is completely secure. CrosaCore cannot guarantee absolute security, which is another reason not to send sensitive health information through general channels.
10. Your choices and rights
You can:
- Choose not to provide information, though this may limit CrosaCore’s ability to respond
- Ask what information CrosaCore holds about you
- Ask CrosaCore to correct information that is inaccurate
- Ask CrosaCore to delete information, subject to any legal or professional retention requirements
- Withdraw text messaging consent at any time
Depending on where you live, you may have additional rights under state privacy law. To make a request, contact CrosaCore using the details in section 14. CrosaCore will respond within the time required by applicable law and may need to verify your identity first.
CrosaCore does not sell personal information and does not use it for targeted advertising.
11. Text messaging and SMS data
If you consent to text messages, CrosaCore records your phone number, the consent you gave, and the date it was given.
CrosaCore does not sell or provide SMS opt-in records, consent records, or mobile telephone numbers to third parties for their independent marketing.
You can opt out at any time by replying STOP, or by contacting CrosaCore directly. Opting out of messages does not cancel an appointment or end care.
Ordinary text messages do not provide the protections of a secure clinical system. Please keep texts to scheduling and administrative matters. Full messaging terms are in the Website and Messaging Terms.
12. Children
This website is intended for adults. CrosaCore does not knowingly collect personal information from children under 13 through this website. If you believe a child has provided information, contact CrosaCore and it will be deleted.
13. Changes to this policy
CrosaCore may update this policy when website functions, service providers, or legal requirements change. The current version displays its revision date at the top of this page.
