These Website and Messaging Terms apply to the CrosaCore website, online forms, scheduling pathways, and text-messaging programs.

The website is operated by CrosaCore LLC, doing business as CrosaCore. References to “CrosaCore,” “we,” “us,” and “our” refer to CrosaCore LLC.

By accessing or using the website, you agree to these Terms and acknowledge the CrosaCore Privacy Policy. Different agreements may apply when you schedule or receive professional services.

Part one: using the CrosaCore website

1. Purpose of the website

CrosaCore operates a private physical therapy practice in Austin, Texas. The website provides information about Modern Physical Therapy, Pain Confidence Consultations, physical therapy evaluations, in-clinic physical therapy, in-home physical therapy, rehabilitation and return-to-exercise services, educational resources, and scheduling and administrative communication.

Website content is intended to help visitors understand CrosaCore and make informed decisions about possible next steps. Service availability, appointment times, locations, and pricing may change.

2. Educational information is not individualized care

Articles, videos, exercise examples, illustrations, downloads, and other website materials are provided for general educational purposes. Website content is not:

  • A physical therapy evaluation
  • Medical advice specific to you
  • A diagnosis
  • Medical or psychological treatment
  • Medical clearance
  • An emergency assessment
  • A substitute for care from a professional who has evaluated you

Reading the website or using an educational resource does not establish what a symptom means in your individual situation.

3. When a provider-patient relationship begins

Visiting the website, joining a mailing list, following CrosaCore on social media, submitting an administrative form, or scheduling a Pain Confidence Consultation does not automatically create a provider-patient relationship.

The Pain Confidence Consultation is a general fit and next-step conversation. It is not a physical therapy evaluation, treatment session, diagnosis, emergency screening, or guarantee that CrosaCore will accept someone for care.

A provider-patient relationship begins only after CrosaCore agrees to provide professional care, required consent and intake documents are completed, applicable financial terms are accepted, and professional services begin.

4. Emergencies and time-sensitive concerns

CrosaCore is not an emergency service. Do not use the website, email, text messages, social media, scheduling tools, or routine portal messages for emergencies or concerns requiring immediate attention.

For a medical emergency, call 911 or seek appropriate emergency care.

New, severe, progressive, or meaningfully different symptoms may require timely evaluation by an appropriate licensed healthcare professional. Routine electronic communications are not continuously monitored.

5. Exercise and movement content

Exercise and physical activity involve risk. General information cannot account for every person’s medical history, current condition, symptoms, medications, physical capacity, exercise experience, equipment, environment, technique, or individual risk factors.

Seek individualized guidance when appropriate. Stop and obtain appropriate assistance when an activity produces a concerning response or meaningful change in your condition.

Pain or discomfort does not always indicate tissue damage or danger, but the website cannot determine the significance of a symptom for a particular visitor.

6. Scheduling and service requests

The website may allow visitors to request or schedule services. Submitting a form or selecting an appointment does not guarantee acceptance for care, appointment availability, clinical appropriateness, a particular service recommendation, a particular outcome, or insurance reimbursement.

CrosaCore may recommend an evaluation, another healthcare professional, personal training, self-management, medical assessment, delayed care, or no CrosaCore service.

7. Pricing and payment

The website may publish the price of selected entry services, such as a consultation or initial evaluation. Ongoing care may be priced after CrosaCore learns more about factors such as the person’s goals, clinical needs, service location, visit structure, equipment requirements, need for progression or reassessment, and the level of support likely to be useful.

CrosaCore will provide the applicable price and material payment terms before a person agrees to ongoing care. Payments may be processed through Square or another payment method specifically identified by CrosaCore.

Separate financial policies may address payment timing, payment plans, cancellations, missed appointments, refunds, pauses, expiration dates, early completion, and superbills.

8. Insurance and superbills

Unless CrosaCore expressly states otherwise, CrosaCore does not guarantee insurance coverage, out-of-network reimbursement, acceptance of procedure or diagnosis codes, application of deductibles, or payment by an insurance plan.

A superbill is documentation that a client may submit to an insurer. It is not a promise of reimbursement. Clients are responsible for verifying their own benefits and insurer requirements.

9. Outcomes are individual

Pain, rehabilitation, performance, and recovery differ among individuals. CrosaCore does not guarantee complete pain elimination, a specific recovery timeline, prevention of every flare-up or injury, a particular strength or performance outcome, return to an activity by a particular date, insurance reimbursement, or that CrosaCore is appropriate for every person.

Descriptions of desired outcomes represent goals of care, not guarantees.

10. Testimonials and client stories

Testimonials, reviews, and client stories describe individual experiences. They should not be interpreted as promises or typical outcomes.

A testimonial does not establish what another person will experience, how long another person’s care will take, which services another person will need, what another person will pay, or whether another person will achieve the same result.

CrosaCore will use client names, images, videos, or treatment-related stories publicly only with appropriate separate permission.

11. Ownership of website content

Unless otherwise identified, CrosaCore owns or licenses the content displayed on the website. This may include written content, logos and brand elements, photographs, videos, graphics and illustrations, educational materials, downloads, page designs, and the Rebuild Method terminology and original materials.

Visitors may read, download, or print a reasonable amount of content for personal, noncommercial use. Without written permission, visitors may not republish substantial website content, sell or commercially distribute CrosaCore content, present CrosaCore materials as their own, remove copyright or ownership notices, copy the overall design of the website, use CrosaCore branding misleadingly, systematically scrape or download website content, or use CrosaCore materials to market a competing commercial service except where permitted by law.

Nothing in this section limits legally protected fair use or other rights that cannot be restricted.

12. Content submitted by visitors

Visitors retain ownership of original material they submit. By submitting an administrative inquiry, the visitor gives CrosaCore limited permission to receive, store, review, and respond to the information for the purpose for which it was submitted.

Submitting a review, photograph, video, testimonial, or personal story does not automatically give CrosaCore permission to publish it. Public marketing use requires appropriate additional permission. Visitors should not submit content they do not own or have authority to provide.

13. Appropriate website use

Visitors may not attempt unauthorized access to the website or connected systems, interfere with website security or operation, introduce malicious software or code, impersonate another person, submit false or fraudulent information, use forms for spam or unrelated solicitation, harass or threaten others, violate another person’s privacy or intellectual-property rights, upload clinical records through a form not intended for secure clinical communication, or use automated tools in a way that unreasonably burdens the website.

CrosaCore may restrict access or take protective action when misuse is suspected.

14. Third-party services

The website may connect to independent services used for appointment scheduling, payments, intake forms, clinical records, secure communication, maps, video, exercise programming, email, analytics, and social media.

Those platforms operate under their own terms, privacy practices, accessibility features, and security measures. CrosaCore does not control every feature, interruption, policy, or action of an independent provider. A link to a third-party service does not mean CrosaCore endorses every statement, product, or service associated with it.

15. Website accuracy and availability

CrosaCore works to keep website information accurate and useful. However, information may occasionally be incomplete, outdated, unavailable, or affected by technical errors. CrosaCore may update or remove content without advance notice.

Visitors should confirm time-sensitive information before relying on it, including appointment availability, hours, location, service area, pricing, policies, and insurance information.

16. Privacy

Use of the website is also governed by the CrosaCore Privacy Policy.

Do not place symptoms, diagnoses, medications, medical records, or other sensitive health information in a general website contact form. Protected health information may also be governed by a separate Notice of Privacy Practices when applicable.

17. Website disclaimers and liability

To the fullest extent permitted by applicable law, the general website and its content are provided on an “as available” basis.

CrosaCore does not promise that the website will operate without interruption, that every technical error will be corrected immediately, that third-party services will always remain available, that general information will be suitable for every visitor, or that website content can replace individualized professional judgment.

To the fullest extent permitted by law, CrosaCore and its website service providers will not be responsible for indirect, incidental, consequential, exemplary, or punitive losses arising solely from the use of, or inability to use, the general website.

This section is not intended to waive professional obligations arising from an established provider-patient relationship, limit liability that cannot legally be limited, or change rights provided by healthcare, privacy, professional-licensing, or consumer-protection law.

18. Restriction of website access

CrosaCore may suspend or restrict website access when reasonably necessary to address fraud, security threats, harassment, unlawful activity, malicious technical activity, or material violations of these Terms.

Restriction of website access does not automatically terminate an established provider-patient relationship. Any clinical relationship must be handled according to applicable professional and legal requirements.

Part two: CrosaCore text-messaging program

19. Types of text messages

CrosaCore may operate two distinct text-messaging categories.

Appointment and service messages may include appointment confirmations, appointment reminders, rescheduling notices, cancellation notices, form reminders, preparation instructions, administrative support, and responses to client-initiated messages.

Marketing messages may include new educational resources, practice announcements, service information, events, offers, promotional messages, and general CrosaCore updates.

Marketing-text consent is optional and is not a condition of purchasing services or receiving care. Appointment-related consent and marketing consent are collected separately.

By selecting the relevant unchecked consent box or otherwise providing valid consent, a person authorizes CrosaCore and its messaging vendors to send the described category of messages to the telephone number provided. When disclosed during sign-up, messages may be delivered using automated technology.

The person providing consent represents that they are the subscriber or authorized user of the number, that the number is accurate, that they are at least 18 years old or have appropriate legal authorization, and that they will notify CrosaCore if the number is changed or reassigned.

Message frequency varies depending on appointments, communication preferences, and interaction with CrosaCore.

21. Message and data charges

Standard message and data rates may apply. Wireless carriers establish applicable charges. Questions about a mobile plan should be directed to the carrier.

22. Stopping messages

A recipient may opt out at any time by replying STOP. CrosaCore may send one final message confirming that the opt-out request was processed.

A person may also request an opt-out by contacting CrosaCore at modernpt@crosacore.com, (737) 383-9566, or another communication method specifically designated by CrosaCore.

Opting out of marketing texts does not cancel an appointment or terminate care. Depending on the technology used, CrosaCore may need to communicate appointment information through email, telephone, or another approved method after a text opt-out.

23. Getting help

Text messaging should not be used for urgent or emergency concerns.

24. Message delivery

Delivery depends on factors outside CrosaCore’s direct control, including wireless-network availability, carrier systems, device compatibility, number accuracy, filtering, and technical interruptions. CrosaCore cannot guarantee immediate or uninterrupted message delivery.

25. SMS privacy

CrosaCore does not sell or provide SMS opt-in records, consent records, or mobile telephone numbers to third parties for their independent marketing.

CrosaCore may provide information to vendors that help deliver or manage messages, including messaging providers, telecommunications companies, aggregators, scheduling systems, and technical-support providers. These vendors receive only the information reasonably needed to perform their functions.

26. Health information and texting

Ordinary text messages may not provide the same protections as an approved clinical-record or secure client-communication system.

Do not send detailed information by ordinary text concerning symptoms, diagnoses, medical history, medications, treatment records, imaging, insurance documents, or other sensitive health information.

CrosaCore may use texts for limited appointment and administrative communication. Sensitive clinical information should be sent only through the communication method specifically designated by CrosaCore.

Part three: updates and contact

27. Changes to these Terms

CrosaCore may update these Terms when website functions, vendors, services, messaging practices, or applicable requirements change. The current version will display its revision date. Where appropriate or legally required, CrosaCore may provide additional notice of a material change.

28. Severability

If one provision is found unenforceable, the remaining provisions will continue to apply to the extent permitted by law.

29. No waiver

A decision not to enforce a provision on one occasion does not permanently waive CrosaCore’s ability to enforce that provision later.
